Elite Health TRT Program

1. Patients understand that purchasing will initiate their prescription being ordered from the pharmacy and delivered to Elite Health Solutions, and subject to the following terms of treatment.

2. Once the patient's prescription is received; the patients will be notified, and they can make an appointment for their first injection.

   • At their first injection, patients can receive an in-office injection (given by staff) or demonstrate to the staff that the patient can self-administer their injections at home.​

   • If home injections are approved; pre-filled syringes will be given to the patient for weekly administration for up to 8 weeks.

3. Patients are required to have their blood work repeated at Elite Health in weeks 7-8 to see if adjustments need to be made to their dosage levels.

   • Patients will be responsible for coordinating the pick-up of their medication (at-home injections) with clinic staff through an appointment.​​

4. If at any point, the patient's blood work shows that their levels are outside of therapeutic levels they will be required to have a telehealth visit with Elite Health's practitioner ($35 cost), by appointment, to discuss the findings and treatment options.

5. If patients are non-compliant with blood work or follow-up appointments, if deemed medically necessary, they will no longer be able to receive injections and/or medication until such time they are seen.
